Specialized Residential Supported Independent Living Day Programming

Introduction:

Minimize time between referral/authorization and initiation of service: The target level of 14 days or less was met agency-wide (3.5 days), and by all services.

Maximize overall consumer satisfaction: The target level of 90% was met agency-wide (90%), and by all services with the exception of Specialized Residential, which scored 83%. Corrective action information is available in the full report.

Maximize consumer community inclusion: The target level of 7 activities each was met agency-wide (10.43 activities a month per person), and by all programs. Specific data is - Specialized Residential (10.83), Supported Living (12.38), and Day Programming (8.83 activities).

Maximize level of staff training: Staff training scored below the target level of 95% agency wide (83%) and for all programs reporting. Corrective action plan is available in the full report.

Maximize physical condition of facilities: Our overall total was 85.7%. Program Supervisors have done individual corrective action plans by location.

Maintain community based living (percentage of days): The target level of 95% was met agency-wide, and by each individual program.

Maintain community based living (percentage of individuals): This objective measures the maintenance of community based living, but the data is measured in people rather than in days. The target level of 95% was met both as an agency (97.7%), and by all programs providing specific data.

Maximize consumer progress in personal goals: Progress toward individual objectives in Day Programming is measured by noting progress during person centered planning meetings, and during the quarterly reviews of the plan. The target level of 85% was exceeded with a score of 95.8%.
